Aged Care Services are a special interest for Optimal SP. We provide mobile and telehealth services to people living independently in the community. We pride ourselves on working well with all members of the aged care team to provide relevant and realistic assessment and treatment services, including:
- Assessment and management of swallowing difficulties associated with normal ageing, dementia and chronic obstructive pulmonary disease
- Facilitation of self, family and carer-driven treatment programs
- Self, family and carer training in identification and management of swallowing problems
- Education in supporting the unique needs of clients with communication impairment
- Periodic review for clients suffering from degenerative conditions
- Regular liaison with general practitioners or geriatricians to maximise cohesive management
